The truth is that both dermal fillers and the way they are used have evolved significantly over time. Advances in filler products, a deeper understanding of facial anatomy, and a more conservative approach mean modern dermal filler procedures can be very different from what many people associate with the word “filler”, especially when you choose your aesthetic doctor carefully.
The way I approach dermal fillers is less about adding volume everywhere, and more about subtle support, structure, and restoring balance to the face. When used correctly, fillers shouldn’t change how you look, they should support your existing facial anatomy to help you look fresher, more rested, and reduce visible signs of ageing.
What Are Dermal Fillers and How Do Dermal Fillers Work?
Dermal fillers are injectable treatments made from hyaluronic acid, a substance that naturally occurs in the body. Hyaluronic acid plays an important role in maintaining hydration and supporting the structure of the skin, which is why it is widely used in aesthetic treatments and skincare.
As we age, we naturally lose collagen, fat, and bone support in the face. This can lead to hollowness, shadows, and changes in facial shape that make us look tired or older than we feel. Dermal fillers work by restoring volume and adding hydration in specific areas of the face.
Modern dermal filler treatments focus on supporting facial structure, improving balance, and working with the natural anatomy of your face. When placed carefully and conservatively, fillers can help restore what has been lost over time without creating an overfilled or artificial appearance. Essentially, when I use fillers, I’m using them to restore what once was and bring back a youthful appearance, rather than to change the way your face look.
What Are Dermal Fillers Used For?
Dermal fillers can be used to address a range of concerns, depending on individual facial structure and ageing patterns. The goal isn’t to change how you, but to subtly enhance and support your natural features.
Some of the most common reasons patients consider dermal fillers include:
Under-eye hollowness (tear-trough filler): those dark circles under your eyes are often caused by hollowness and no amount of sleep will get rid of them, under-eye filler (or tear-trough filler) can be used to restore the lost volume and improve the appearance of dark circles, fine lines, and wrinkles.
Loss of facial volume: over time we lose volume, especially in the mid-face as part of the natural ageing process. Carefully placed filler can support restoring the volume loss and adding hydration to the face. This can be particularly good for people who are seeing jowls, sagging skin in the lower face, and lack of definition in the cheeks.
Deep lines or shadows: when lines are caused by structural changes rather than surface wrinkles, filler can be placed to restore the volume and in turn reduce the appearance of deep lines and facial shadows.

When dermal fillers are done well, people often look fresher and more rested rather than noticeably “done”. Friends or family may comment that you look well, without being able to pinpoint exactly why – that’s exactly why you won’t see lots of really contrasting dermal fillers before and after on my social media or website, my approach is always to maintain your own facial structure with results that are undetectable.
What Dermal Fillers Shouldn’t Be Used For
Dermal fillers are not the right solution for every aesthetic concern, and part of responsible practice is knowing when not to use them.

Rather than aiming for dramatic changes in a single session, a gradual, long-term approach often leads to more natural looking dermal fillers and sustainable results over time.
Are Dermal Fillers Safe?
A question I often get asked during filler consultations is “are dermal fillers safe?”. If carried out by a qualified and experienced practitioner, dermal fillers are considered a safe and well-established treatment.
Safety depends not only on the product used, but on the practitioner’s knowledge of facial anatomy, patient assessment, and appropriate injection technique. Who carries out your treatment matters just as much as the product itself.
