Masseter Botox Treatment: The Secret to Jaw Slimming & Relief from Teeth Grinding
Masseter Botox is one of those treatments that surprises people. Most patients who come to me for Masseter Botox injections aren’t primarily looking for a cosmetic result, instead they come because they are in pain or discomfort. They wake up with jaw tension, headaches, or a dull ache in their face, and they’ve often been told by their dentist that they’re grinding their teeth at night without even realising it.
What surprises them is that the same treatment that relieves that discomfort can also, gradually, slim and soften the lower face.
In this blog, I’ll cover everything you need to know about masseter Botox: what it is, how it works, who it’s suitable for, and what to realistically expect.
What Is the Masseter Muscle?
The masseter is the large muscle that runs along the side of your jaw. It’s one of the strongest muscles in the body relative to its size, and its primary function is chewing. If you place your fingers at the back of your jaw and clench your teeth together, you’ll be able to feel it working - that bulge is your masseter contracting.
For most people, the masseter works perfectly well and causes no issues. But for others, particularly those who clench or grind their teeth, either during the day under stress or at night during sleep, the masseter can become overdeveloped and chronically tense. This leads to a condition commonly known as bruxism.
What Is Bruxism, and Could You Have It?
Bruxism is the medical term for habitual teeth clenching or grinding. It’s more common than many people realise, and a significant number of people with bruxism aren’t even aware they do it, because it often happens during sleep.
Some of the signs that you might be grinding or clenching your teeth include:
Waking up with jaw pain, tightness, or a dull ache
Frequent headaches, particularly in the morning or at the temples
Tooth sensitivity or worn-down tooth surfaces (often flagged by a dentist)
Clicking or discomfort in the jaw joint (TMJ)
A visibly wide or square lower face due to muscle bulk
Earache without any ear-related cause
If any of these sound familiar, it’s worth discussing with a medical professional (like me). Bruxism can have a real impact on quality of life, both physically and in terms of dental health over time.
What Is Masseter Botox?
Masseter Botox involves injecting small amounts of Botox (botulinum toxin) directly into the masseter muscle on each side of the face. The same principle that makes Botox effective for softening facial lines (temporarily reducing muscle activity) applies here too, just in a much larger and deeper muscle.
By reducing the overactivity of the masseter, the treatment achieves two things:
1. Therapeutic relief The excessive tension in the muscle is reduced, which eases jaw pain, headaches, and the discomfort associated with bruxism and TMJ issues. Many patients describe this as one of the most noticeable and immediate benefits.
2. Gradual jaw slimming When a muscle is less active over time, it gradually reduces in size, in the same way that a muscle you stop exercising becomes less prominent. For patients whose masseter is overdeveloped due to chronic clenching, this means the lower face can become gradually slimmer and more softened in appearance. This happens over weeks to months, not immediately.
It’s worth noting that while masseter Botox can slim the jaw, it’s not a dramatic reshaping procedure. It works subtly, over time, and the results depend on the degree of muscle overdevelopment to begin with. For patients with significant masseter bulk due to habitual clenching, the change can be quite noticeable. For others, the benefit is primarily therapeutic rather than cosmetic.
How Long Does It Take for Masseter Botox to Work?
This is one of the most common questions I’m asked, especially from patients in a lot of pain from Bruxism, so it’s worth setting expectations clearly.
The therapeutic effect (relief from jaw tension and headaches) tends to become noticeable within one to two weeks of treatment, as the muscle begins to relax.
The cosmetic effect (slimming and softening of the jaw) takes longer. Because the muscle reduces in size gradually as it becomes less active, most patients notice visible changes between four to eight weeks after treatment, with full results at around three months.
Results typically last between four and six months, sometimes longer with repeat treatment. Over time, many patients find that regular treatment keeps the muscle consistently reduced and the therapeutic benefits ongoing.
Where Is Masseter Botox Injected?
Masseter Botox injections are placed directly into the masseter muscle on each side of the face. I assess the size, shape, and activity of the muscle during your consultation, and the injection sites are chosen based on your individual anatomy.
The treatment itself takes around 15 to 20 minutes. Most patients describe the discomfort as minimal, a brief stinging sensation at each injection point, which passes quickly. There is no surgery, no incisions, and no anaesthetic required, though I take every step to make the experience as comfortable as possible.
There is no meaningful downtime. You can return to normal activities immediately, though I advise avoiding strenuous exercise for 24 hours and not rubbing or applying pressure to the treated area for a few hours after treatment.
Is Masseter Botox Just for Teeth Grinding?
No, though it is one of the most common reasons patients seek it out. There are broadly three groups of people who benefit from masseter Botox:
Those with bruxism or jaw tension The primary reason many patients come to me for this treatment. Relief from jaw ache, headaches, and TMJ discomfort can be significant and life-improving.
Those with facial slimming goals Patients who feel their lower face looks wide, heavy, or overly square, whether due to muscle bulk, genetics, or habitual clenching, can use masseter Botox for jaw slimming and see a meaningful softening of the jaw contour over time.
Those with a combination of both Very often, the two overlap. A patient who has been grinding their teeth for years may have an overdeveloped masseter that is both causing them discomfort and contributing to a wider lower face. Treating one addresses the other.
What About Jaw Clenching During the Day?
Daytime clenching is often stress-related and many people do it without realising during concentration, commuting, or difficult conversations. If you notice yourself clenching your jaw, holding tension in your face, or frequently getting tension headaches, it may well be that your masseter is working harder than it needs to.
Masseter Botox can help with daytime clenching in the same way it helps with night-time grinding, by reducing the muscle’s ability to contract with excessive force, which in turn reduces the tension and discomfort.
Masseter Botox Before and After: What to Expect
The before and after results for masseter Botox are often subtle and very natural. Dramatic transformations are possible, but they’re most pronounced in patients who have significant masseter hypertrophy (overdevelopment) to begin with.
For most patients, the visible result is a gradually softer, more oval lower face, less square, less heavy-looking. This can complement the overall facial balance, particularly for those with a naturally strong or angular jaw.
For patients whose primary concern is therapeutic, the “before and after” often plays out not in photographs but in how they feel - fewer morning headaches, less jaw tension, better sleep quality, and in some cases, improvement in teeth sensitivity flagged by their dentist.
I always encourage patients to have realistic expectations and to discuss what’s achievable for their specific anatomy during consultation.
Is Masseter Botox Safe?
When carried out by a qualified, medically trained practitioner, like myself, masseter Botox is considered safe and well-tolerated. It has a well-established track record in both aesthetic and medical settings.
As with all injectable treatments, there are potential side effects to be aware of:
Mild swelling or tenderness at the injection sites, which usually settles within 24 to 48 hours
Occasional bruising
Rarely, temporary asymmetry if the muscle responds differently on each side, this will be reviewed and adjusted at a follow-up appointment
One concern patients sometimes raise is whether masseter Botox affects their ability to chew. In the doses used for aesthetic and therapeutic treatment, it does not. You may notice that very forceful chewing, biting into a very hard food, for example, feels slightly different as the muscle relaxes, but normal eating is unaffected.
